ACT NOW ADVOCACY TRAINING PROGRAM
AUGUST SERIES
PATIENT SUBMISSION REGISTRATION
We are thrilled to offer you participation in our Advocacy Connection Team (ACT Now). This series is designed to educate and train Migraine & Headache Fellows along with a patient or caregiver, to incorporate and encourage Advocacy in every Migraine & Headache Disorder Treatment Plan. Submission is required and confirms your active commitment to attend virtual monthly series sessions with your partner fellow (sessions will be recorded in the event you are unable to attend the live presentations), engage in guided conversations in a private Facebook Group and partner with a team to complete an advocacy project of your choice. Please submit this consideration registration form upon invitation to participate. Full program details may be found here.
A series of 8 meetings will be held monthly at 8PM ET beginning in August. Final dates to follow. All meetings are recorded, but we expect you to vitually attend at least 5.
