We are raising money for the Seniors Care Program at Humber River Hospital Foundation!
The funds raised will go towards programs including:
Humber River Hospital Dementia Initiative - Humber is teaching frontline staff how to provide the best quality care for patients with Dementia
Preview Emergency Department - This program focuses on providing patient care before the patient needs to be transferred to the Emergency Department for more critical care.
Scope+ For Seniors - SCOPE+ is a platform that promotes collaborative work between primary care, hospital services, and community health partners to serve patients with complex needs.
