Welcome to Community Health Center's fundraising page for the Numbers for Nonprofits Program at the 2024 Falmouth Road Race!
The mission of Community Health Center is to improve the health of our community through the provision of equitable, patient-centered, high quality, compassionate health care to all, regardless of ability to pay. Community Health Center provides care within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients in overcoming affordability, financial, linguistic, cultural, transportation and other barriers to care. Community Health Center has offices in Mashpee, Falmouth, North Falmouth, Bourne, Sandwich and Centerville. You can learn more at www.CHCofCapeCod.org.

Thank you for visiting our page, and for considering supporting the CHC team in the 2024 ASICS Falmouth Road Race. I am proud to be able to run and fundraise to support Community Health Center, and their efforts to support increased access to integrated care services for more than 25,000 individuals on the Cape, Islands and beyond.
Your support is so welcome and appreciated. It will help me to reach my fundraising goal to participate in the August 18 event, and it will support an outstanding cause in CHC.
