Description

Duffy Health Center is a 501(c)(3) nonprofit healthcare center that treats more than 3,200 Cape Codders each year
who are homeless or at risk of homelessness.
Duffy Health Center provides skilled and compassionate primary care, mental health care, and substance use disorder treatment to restore health and rebuild lives for a stronger Cape Cod. We envision a Cape Cod where all persons have access to quality healthcare, safe and stable housing, and lives filled with connection and purpose.
In addition to the daily care we provide patients in our Hyannis health center, Duffy operates several critical healthcare and support programs:
REACH Mobile Unit The REACH Mobile Unit is a strategic priority for Duffy, providing recovery support navigation, showers and basic hygiene, access to medication for opioid use disorder, and connections to healthcare, case management, and other critical services for the most marginalized residents of Barnstable County. The REACH Mobile Unit brings these services to five Cape Cod towns each week.

Moms Do Care The Moms Do Care program supports pregnant and parenting women who are seeking recovery from a history of any substance use. Program staff include a Certified Addiction Registered Nurse, a Licensed Therapist, and Peer Recovery Coaches who have lived experience. We assist in accessing Medication Assisted Treatment and healthcare services, and support women in their recovery after birth with case management, counseling, and access to resources. Services are provided for moms by moms who have walked this path during their own pregnancy.
-----------------------------------
Cape Teens Rising The Cape Teens Rising program is a 6-month outpatient treatment program for Cape Cod teens struggling with substance use challenges. Teens in the program face their challenges with the expert guidance of a Therapist, a Recovery Coach, and a Peer Support Staff with lived experience. During their twice-weekly sessions, teens talk to their peers who are going through similar challenges, create supportive bonds, and learn about how others navigate their recovery. The program is built to help teens achieve their goals and celebrate successes along the pathway of recovery.
-----------------------------------
In From the Streets In From the Streets is a community-wide program which offers temporary motel shelter and meals to homeless individuals during extreme weather temperatures or who are medically frail. Duffy’s integrated care model, skilled case managers, and timely response keep vulnerable people within the safety net of care. Last year, Duffy provided 5,828 nights of stay to 149 members of the homeless population.
-----------------------------------
Medical Respite Medical Respite provides short- and medium-term housing for homeless patients recently discharged from the hospital which includes housing, medical care, healthy food, and spiritual care, if desired. The respite program ensures that patients recover from their hospital stay in a safe environment with daily medical support, improving health outcomes and reducing the recurrence of a hospital stay.
Your support of Duffy Health Center directly and positively impacts the most vulnerable members of our community as we support them in reaching their goals, restoring their health, and rebuilding their lives.
THANK YOU. YOU MAKE A DIFFERENCE.
