Journey Home Hospice, a program of the Saint Elizabeth Foundation, provides 24/7 residential hospice palliative care for patients experiencing homelessness and structural vulnerability at end-of-life. With thirteen beds in downtown Toronto, we are helping to answer the question: if most Canadians want to die at home, where do you go to die when you don't have a home?
