Raise money for Community Health Center
The mission of Community Health Center is to improve the health of our community through the provision of equitable, patient-centered, high quality, compassionate health care to all, regardless of ability to pay. Community Health Center provides care to more than 25,000 individuals annually within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients in overcoming affordability, financial, linguistic, cultural, transportation and other barriers to care.

Your support will enable CHC to achieve its mission to improve the health of our community through the provision of equitable, patient-centered, high quality, compassionate health care to all, regardless of ability to pay.
Your donation will allow CHC staff to remain relentless in addressing the social determinants of health that impact individuals in our community, including housing insecurity, food insecurity, transportation challenges, employment challenges, language barriers, and access to affordable health care.
